State Filing Guide
The State DOI Filing module tracks all regulatory submissions to state Departments of Insurance (DOI) across eKlotho Inc's operating markets.
  • Compliance Score — % of filings submitted on time vs. total due in the period
  • Overdue — Past due date with no submission; requires immediate action
  • Pending — Due date approaching; filing not yet prepared
  • In Review — Submitted to DOI, awaiting response
  • Accepted — DOI has approved or acknowledged the filing

  • Rate Filing — Annual or quarterly premium rate submissions. Requires actuarial certification. Filed via SERFF in most states.
  • Form Filing — Member-facing document updates: enrollment forms, EOBs, SB summaries. State-specific language compliance required.
  • Network Adequacy — Demonstrates sufficient provider coverage by specialty and county. CMS and state standards may differ.
  • Financial Statement — Statutory NAIC financial reports. Typically due March 31 for prior year.
  • Annual Report — Comprehensive plan report covering enrollment, quality, and operations.
  • Click File Now on overdue rows to open the detail panel immediately.
  • Upload all required documents before submitting to the state DOI portal.
  • Contact the state DOI to explain the delay and request an extension if applicable.
  • Document the reason for delay in filing notes for audit trail purposes.
  • Most states charge $100–$1,000/day in civil monetary penalties for late filings.
  • After submitting, record the DOI confirmation number in the filing record.
The Bulk Status Check queries each state's DOI or SERFF portal to pull the latest status for all submitted filings.
  • Run before weekly compliance meetings to ensure data currency.
  • New accepted/rejected statuses trigger notifications to assigned officers.
  • Not all states support API status checks — CO and NM may require manual verification.
  • Results are timestamped; check the Last Updated indicator after running.
