Understanding Enrollment Status

Enrolled patients are those who have consented to CCM or RPM programs.

  • Active — Currently participating and billing-eligible
  • Pending — Consent obtained, awaiting first contact
  • Inactive — Temporarily paused or withdrawn
  • CCM and RPM statuses are tracked independently
CCM Program Requirements

Chronic Care Management (CCM) requires:

  • Two or more chronic conditions expected to last ≥12 months
  • Minimum 20 minutes of clinical staff time per month
  • Comprehensive care plan created and maintained
  • 24/7 access to care team for urgent needs
  • Patient written consent on file
RPM Program Setup

Remote Patient Monitoring (RPM) workflow:

  • Assign appropriate monitoring device (BP, scale, glucose meter)
  • Device must transmit ≥16 days of data per 30-day period
  • Clinical staff must review and respond to alerts within 24 hours
  • Monthly billing requires data collection threshold met
Managing Disenrollment
  • Patient may withdraw consent at any time
  • Document reason for disenrollment in care notes
  • Final billing period ends at month of withdrawal
  • Devices must be retrieved or deactivated within 30 days
