About UDS Reporting ▾

The HRSA Uniform Data System (UDS) is an annual reporting requirement for all FQHCs. It collects data on patient demographics, services, staffing, financials, and quality measures.

Completeness Scores ▾
  • Green (≥90%): Table is ready for submission.
  • Amber (75–89%): Review required before submission.
  • Red (<75%): Significant gaps — do not submit until resolved.
Submission Deadline ▾

The UDS must be submitted via HRSA Electronic Handbooks (EHBs) by February 15 annually. Late submission may affect grant renewal. Allow 2–3 weeks for final data validation.
