This dashboard provides a real-time view of your care coordination team's workload, case acuity, overdue actions, and recent clinical activity.

  • Open Cases — All active cases regardless of status
  • High-Risk — RAF ≥ 1.5 or clinical alert triggered
  • Active Transitions — Members currently in a care transition (admit/discharge)
  • Overdue Actions — Tasks past their due date

CMs are flagged amber when caseload exceeds 24. Recommended maximum is 30 cases per CM. Use the Case Management page to reassign cases when balancing is needed.

Overdue tasks are escalated to the CM's supervisor after 48 hours. Post-discharge calls are highest priority — CMS STAR Ratings measure 30-day follow-up rates.
