The Care Team page lists all healthcare providers involved in a patient's care. Each card shows contact details, specialty, and the practice they belong to.

  • PCP — Primary Care Physician, main coordinator
  • Care Manager — Oversees CCM program
  • Cardiologist — Specialist for cardiac conditions
  • Behavioral Health — Mental health specialist
  • Care Coordinator — Coordinates appointments & referrals

Click "Add Team Member" to assign a new provider. You can search by name, NPI, or specialty. The provider must be credentialed in the network before assignment.

Use the phone icon to call, the email icon to compose a message, or the message icon to send an internal note. All communications are logged in the patient record.
